Form is available in Massachusetts and expanding to additional states soon. Our $150 monthly fee includes all interactions with your medical doctor, unlimited interactions with your registered dietitian, access to Form Health's software platform, educational materials, and more.

Florencia Halperin, MD, MMSc

Dr. Halperin is an endocrinologist who is passionate about caring for people with obesity, and improving their access to the most effective weight loss strategies available today. Dr. Halperin was the Founding Co-Director of the Center for Weight Management and Metabolic Surgery at Brigham and Women’s Hospital and continues to see patients there part-time. She is also on faculty at Harvard Medical School.

Julia Axelbaum, RD, CSOWM

Julia is a Registered Dietitian board certified in obesity and weight management. She has her nutrition degree from New York University and completed her clinical training at Beth Israel Deaconess Medical Center. Julia works one-on-one with patients to help them learn how to take control of emotional eating, develop a more balanced mindset and improve their relationship with food.
